| The Enrollment Form is mandatory before attending the ECCA After School Program. However, it does not guarantee your child a spot. We will do our best to meet all requests. Be sure to fill out your expected After School schedule at the end of the form. |

| Afterschool Program Fees |
| $16.00 for each normal day (3:15pm-5:55pm) a child attends ECCA's After School Program. |
| $32.00 for each half day / noon release (12:00pm-5:55pm) |
| Students are charged to their SchoolPay.com accounts under the Meals tab. Please be sure to load money onto these accounts prior to the students' attendance in After School. |

| Pick Up Procedures Parents or other authorized person are responsible for signing their child(ren) out of the program at the end of each day. Children will only be released to those authorized by the parent/guardian on the registration form. It is the parent’s responsibility to notify the ECCA After School program of any changes to authorization. Those picking up children should be prepared to show identification to staff members upon request. No child will be released to anyone who is not authorized to pick up that child. |
| Please enter the school through the Kindergarten doors (to the right of the playground directly in front of the school). Follow the signs to the After School room. Outside the room you will find the Sign Out binder. Please sign out your child(ren) each day. |
| How do I know which days my child(ren) is/are signed up? |
| A schedule will be pushed out to parents each Wednesday for the following week. Siblings will be grouped together. Please give 24 hour advance notice of cancellations or your account will still be charged! |
